Whether you choose Original Medicare or Medicare Advantage, most people pay a monthly Part B premium to Medicare. Here's what to expect in 2026.
Most beneficiaries pay the standard monthly Part B premium (set by Medicare each year). This is deducted from your Social Security check if you receive benefits, or billed quarterly if not.
If your income is above certain thresholds, you may pay an Income-Related Monthly Adjustment Amount (IRMAA) on top of the standard premium. IRMAA is based on your tax return from two years prior.
After meeting the annual Part B deductible, you typically pay 20% coinsurance for doctor visits and outpatient care under Original Medicare. Medicare Advantage plans replace this with copays and an out-of-pocket maximum.
If you don't sign up for Part B when first eligible and don't have qualifying employer coverage, you may pay a permanent 10% penalty for each 12-month period you delayed. A licensed agent can review your timeline.
